Our Expert Oral Surgeons
Our oral surgeons, Dr. Parmer, Dr. Macholl, and Dr. Siew, bring extensive expertise to the field of oral and maxillofacial surgery, which addresses a wide range of conditions affecting the mouth and facial regions. All our doctors have gone to medical school and hold MD degrees along with having completed 6 years of hospital-based training beyond dental school, specializing in surgery of the mouth, jaws, and face as well as the safe administration of intravenous (IV) sedation and general anesthesia. Their expertise spans dental implant surgery, wisdom tooth removal, corrective jaw surgery, jaw reconstruction, facial trauma repair, and oral pathology treatment.








David Parmer,
DDS, MD
Dr. David Parmer, DDS, MD, grew up in the west Texas town of Levelland and attended Texas Tech University. He received his Doctor of Dental Surgery from the University of Texas Health Science Center in San Antonio in 1991. After finishing dental school, he served as a dentist in the US Air Force for three years. His first year, he completed a general dental residency at Langley AFB in Virginia. His last two years, he served as a general dentist at Dyess AFB in Abilene.
Dr. Parmer returned to San Antonio for his oral & maxillofacial surgery training. He started in medical school and received his Doctor of Medicine at UTHSC-SA in 1997. He completed his general surgery internship at University Hospital and then finished his oral & maxillofacial residency at UTHSC-SA in 2000.
He is the past President of the Texas Society of Oral and Maxillofacial Surgeons.
Dr. Parmer is a diplomate (board certified) of the American Board of Oral and Maxillofacial Surgery and a fellow of the American Association of Oral and Maxillofacial Surgeons.
He is a member of the Fort Worth District Dental Society, Texas Dental Association, American Dental Association, North Texas Society of Oral and Maxillofacial Surgeons, Texas Society of Oral and Maxillofacial Surgeons, and Southwest Society of Oral and Maxillofacial Surgeons.
Dr. Parmer contributes to programs for the underprivileged, including the Save A Smile program. He has also participated in several medical missions to Mexico.
Dr. David Parmer is married to Dr. Fariba Parmer, who is an optometrist. He enjoys running and traveling, and he spends his free time with their twins.



James Macholl,
DDS, MD
Dr. Macholl was born in Milwaukee, Wisconsin and grew up in the small town of Hubertus, Wisconsin. He graduated from Marquette University School of Dentistry in 1991. After graduation, Dr. Macholl joined the United States Army.
His first assignment was as a resident in general dentistry at Fort Campbell, Kentucky. This was followed by three years of service as a general dentist in Mannheim, Germany. Dr. Macholl was selected by the Army for oral surgery training and sent to the University of Texas Health Science Center in Houston. There, he received his Doctor of Medicine and completed an internship in general surgery. Dr. Macholl finished his residency in oral and maxillofacial surgery in 2001.
Since finishing his residency, Dr. James Macholl has been assigned to Great Lakes Naval Base in Illinois, Walter Reed Army Medical Center in Washington, DC, and Landstuhl Regional Medical Center in Germany. Dr. Macholl also completed one year of training in a cosmetic surgery fellowship. He retired as a Colonel from the US Army after more than 23 years of service. He spent the last eight years of his military career involved in the care of service members wounded in Iraq and Afghanistan. During his time at Walter Reed, he trained oral surgery residents and was the residency program director.
After retiring from the military, Dr. Macholl joined Alliance Oral and Maxillofacial Surgery in 2014.
Dr. Macholl is a diplomate (board certified) of the American Board of Oral and Maxillofacial Surgery. He is a Fellow of the American Association of Oral and Maxillofacial Surgeons.
Dr. Macholl spends his free time with his wife Dora and their four rescue dogs. Dr. Macholl and Dora work with local charities including Global Hands of Healing. Through Global Hands of Healing, they go on annual medical mission trips to Guatemala and Panama.




Stephen Siew,
DDS, MD
Dr. Stephen Siew was born in Seattle, Washington. He earned his undergraduate degree in Neurobiology from the University of Washington, graduating summa cum laude. He went on to receive his Doctor of Dental Surgery (DDS) from the University of Washington School of Dentistry in 2019, where he graduated at the top of his class and was inducted into Omicron Kappa Upsilon, a prestigious national dental honor society.
Dr. Siew relocated to Texas for advanced training in oral and maxillofacial surgery. He earned his Doctor of Medicine (MD) from Texas Tech University Health Sciences Center. He then completed a general surgery internship at Baylor University Medical Center and an oral and maxillofacial surgery residency at Texas A&M Health Sciences Center in Dallas, Texas. During his training, Dr. Siew developed extensive expertise in extractions, dental implants, corrective jaw surgery, facial trauma, and pathology.
In his free time, Dr. Siew enjoys golfing, playing pickleball, exploring new restaurants, and spending time with friends and family. He is proud to be part of a practice that treats every patient like family.
